The Diabetes Management Program at UT Southwestern Medical Center offers a wide variety of educational programs for people with diabetes. Group and individual appointments are available Monday through Friday.

A physician's referral is needed to participate in the program. The referring physician will receive documentation detailing the patient's progress. Insurance benefits will be verified prior to scheduling.
The Diabetes Management Program is located in the Professional Office Building (POB) I Suite 200. We are open from 7:30 a.m. to 4 p.m., Monday through Friday.
